Friends and Family Test results - March 2018

Every month we monitor and review how satisfied patients are with their care. In March 2018, 66% of patients said they were very likely or likely to recommend Priory Avenue Surgery to friends or family.

Access to Medical Records

We are pleased to advise that patients can now request access to their medical records Please see a member of the Reception team for further information. Or complete the application form and take it to Reception.

Please remember to bring two forms of identification with you.  At least one should contain a photo.  Any two of the following documents are acceptable: passport, driving license or a bank statement/ utility bill dated within the last 3 months.


Please let us know beforehand if you cannot keep your appointment and we can allocate your slot to another patient who may be waiting. Many hours are wasted each year because some patients fail to turn up for appointments and do not inform us.

Are your contact details up to date? Have you recently moved or changed your home/ mobile telephone number? If you have, please let the surgery know so that we can update your records.
