The Priory Avenue Surgery

HOW DO I...
OBTAIN TELEPHONE ADVICE?

To avoid interrupting the doctors during consultations - unless the matter is urgent - please phone between 11.30 and 12 noon.

If you wish to speak to the practice nurse about a medical problem, please let the receptionist know and the nurse will telephone you during the morning.
If you request an emergency, same-day appointment or a home visit, someone may contact you to assess the urgency of your condition. This will help us use the appointment time more effectively.

HOW DO I...
MAKE AN APPOINTMENT?

To make an appointment with the doctor or practice nurse:

Either telephone 947 6828 between 8.00am and 6.30pm or call in-person between the same hours.

If you feel that you need to see the doctor on the same day, please tell the receptionist. She will ask you for your telephone number and a brief description of the problem if you are willing to tell her. If your problem is very urgent, please tell the receptionist. The DUTY DOCTOR will telephone you back as soon as they are able. This may be in up to 2 hours. When the DUTY DOCTOR speaks to you they will either give you the advice you need on the telephone or give you an appointment THE SAME DAY.
If you do not need an appointment on the same day you can book an appointment between two days to four weeks ahead. We will try to give you an appointment with the doctor of your choice, but when he or she is not available you will be offered an appointment with another doctor. Nurses’ appointments are all pre-bookable.

When dealing with patients and their sometimes complex problems, the doctors will inevitably run late at times. Please be patient if you are required to wait as it may be you who needs more time when you come to the surgery on another occasion.

MISSED APPOINTMENTS

If you are unable to keep your appointment, please notify the surgery so that we can offer the appointment to another patient.

HOW DO I...
OBTAIN A HOME VISIT?

Home visits are normally reserved for emergencies, the elderly and infirm. Please contact the surgery on 947 2432. Give the receptionist the patient's name, age, address, and 'phone number. It is also helpful to briefly state the reason for the visit in order that the doctors can decide in which order to make their calls.

Please do not call out the doctor unless the patient is obviously unable to come to the surgery. Home visits take the doctor far more time, and sometimes full examinations and tests cannot be done in the home.

HOW DO I...
OBTAIN TEST RESULTS?

Specimens are taken to the hospital at 10.45am and 4.00pm. The times taken for us to receive the reports vary. Blood tests take up to a week, x-rays seven to ten days and cervical smears three to six weeks. You will be notified only if the result is abnormal, except in the case of cervical smears where you will always receive the result, If you wish to know your results, telephone 947 2431 after 11.00am to speak to the receptionist.

HOW DO I...
OBTAIN A REPEAT PRESCRIPTION?

If you are taking regular medication, your doctor may give you a repeat prescription slip. When you need more supplies the slip should be put in the box in the porch or posted to the surgery.

Please allow 48 hours (2 working days) before collection. which is only available on weekdays 8.00am - 6.30pm. If you wish the prescription to be sent to you, attach a stamped addressed envelope to your slip.

TO AVOID ERRORS, THE RECEPTIONISTS ARE NOT PERMITTED TO ACCEPT TELEPHONE REQUESTS.

HOW DO I...
CHANGE MY DETAILS?

If you move, please let us know your new address and phone number. We will normally continue your care unless you move outside our practice area.

HOW DO I...
REGISTER?

Ours is currently a partnership of five doctors, with an additional salaried GP, Dr Catrin Woolley. The map on this website shows the area within which we accept patients.

Please register by filling out the appropriate forms obtainable from reception before you need to see a doctor so that your details can be put on the computer for the doctor’s use.

You will be able to express a preference for the doctor you usually wish to see, although you may see any of the doctors who have an appointment available at the time you wish to attend. We reserve the right to limit the number of patients who express a preference for any individual doctor in order to keep the workload, and hence the availability, of the doctors balanced.

You will be asked to fill in a short questionnaire on your past and current medical history.

TEMPORARY RESIDENTS

A visitor to your home may be treated, if necessary, as a temporary resident, once they have completed the appropriate form. If, however, they intend staying more than three months, it will be necessary to fully register.

This is confidential, and is used to help us to give you the best possible care, as we will not have your medical records immediately.
New patient health checks are available upon request.
