PATIENTS'
CHARTER
The following are the standards
set within this practice for the benefit of our patients. We are here to give
you the most appropriate treatment and advice by the most suitably qualified person.
We consider that we are in partnership with our patients to achieve the provision
of the best possible healthcare for each person on our patient list.
Our Responsibilities
To You
The administrative and nursing staff
will wear name badges. The doctors' consulting rooms will have nameplates
on the doors.
You will not be given any treatment without your informed
consent.
The doctors and nurses will make every effort to keep to appointment
times. If there is likely to be a delay of more than 30 minutes, you will be informed
on your arrival.
We will endeavour to give you an appointment with the
most suitable person for your requirements, and by the most appropriate method.
Often a telephone appointment will be most convenient for both doctor and patient.
Home
visits may be requested if you are terminally ill, disabled or house bound elderly.
We
will do our best to always offer you an appointment within 24 hours, 48 as a maximum,
not necessarily with your registered GP. To achieve this, we limit the number
of pre-bookable appointments.
You will be treated in a polite and respectful
manner by all members of the practice team.
You have the right to see your
records, subject to the limitations in the law. These will be kept confidential
at all times.
We will help you to promote good health and avoid illness,
and offer self-help information for the treatment of minor illness.
We
will endeavour to have repeat prescriptions ready for collection within 48 hours
of request.
Your Responsibilities
To Us
Please make sure your personal contact
details such as telephone and address are always up to date.
Please tell
us and ask questions if you do not understand what is said to you.
Please
let us know as soon as possible if you are unable to keep an appointment, even
if booked for the 'same day'.
Please attend promptly for your
appointment.
Please be willing to answer the receptionist's questions.
All information given will be considered confidential, and she will only ask for
information that we consider necessary to determine who you should see.
Please
request a home visit before 10.30am if possible.
Please treat all practice staff with courtesy.
Please
write to your GP if you wish to see your records. A fee may be charged.
Remember
you are responsible for your own health and that of your children.
We will
give you our professional help and advice - please act upon it.
Please
do not leave your repeat prescription request to the last minute, so that it becomes
urgent.
